Dear Valued Client,Please be advised that when providing a photograph to your process server for the purpose of confirming identification of the individual on whom service is to be effected in a Supreme Court Family Relations file, the process server will note on his/her Affidavit of Personal Service (Form F15) that ” attached to this affidavit and marked as Exhibit B is a photograph that is a true likeness of the person I served and I am informed by …………….[name]……………. that Exhibit B is a photograph of …………….[name of person served]…………….”  As a result, the person who provided the information on the identity of the person served must file an affidavit to confirm that information.  This information can be included in the Affidavit – Desk Order Divorce (Form F38) or a separate Affidavit.  The courts will be looking for this information within your documents and may reject those packages where it is not noted.If you have any questions regarding this update, please contact Dye & Durham’s Litigation Division at 604-257-1850.
